Healthy Connection specialist exercise clinic is exclusively staffed by Accredited Exercise Physiologists who offer exercise prescription across a vast range of clientele for people suffering chronic diseases, musculoskeletal pathologies, cardiac rehabilitation, disabilities and general healthy populations. Participants are prescribed an individual clinical exercise program, based upon the results of a comprehensive health and fitness assessment and previous medical history. Exercise sessions are supervised at all times by an Accredited Exercise Physiologist.

GP Referral

Healthy Connections welcomes referrals from doctors requesting specific exercise physiology treatment for patients with a diagnosed chronic disease. We offer several programs as community based modes of outpatient care. These programs with the appropriate referral pathway are indicated below.

Group Diabetes Exercise and Education Programs
Team Care Plan Referral (Medicare)

Available for patients with Type 2 Diabetes 

Bulk Billed 1 Health Assessment and 8 group exercise and education sessions

Enhanced Primary Care (EPC)
GP Management Plan Referral (Medicare)

Medicare subsidised for up to five individual exercise physiology consultations

DVA Referrals

D904 referral form for white and gold card holders is required prior to commencing services through the Department of Veteran Affairs. Please note the specific condition for white card holders must be listed on the referral.

Healthy Connections is a registered provider of Exercise Physiology services for the NDIS.  We have individual and group services available.  All sessions are individualized and targeted towards management of each participants disability and supporting independence to complete activities of daily living.  Service agreement required before commencement of service, contact staff for more information.

Aged Care

Exercise Physiology can be claimed through aged care funding schemes.  If you are receiving services through a Home Care Package or the Commonwealth Home Support Program, you may be eligible to receive Exercise Physiology services funded through this.  If you are interested in this, discuss with Health Connections or your package manager.
